Not all cancers are the same. Precision medicine is about tailoring treatments to an individual’s cancer. Precision-Panc clinical trials are delivered through the NHS and match people with pancreatic cancer to the trial most likely to work for them.
The Precision-Panc platform was founded in 2017, bringing together expertise from Glasgow, CRUK Institutes (Scotland, Cambridge, Manchester), Institute of Cancer Research in London, University of Oxford and the NHS. There is a UK-wide network of over 30 hospitals that can offer precision medicine clinical trials to patients with pancreatic cancer, based on the genomics of the patient and tumour.
The Precision-Panc platform’s pancreatic cancer research is diverse, focusing on translating basic scientific discoveries into the clinic. These include defining the genetic characteristics of pancreatic cancer, developing biomarkers of prognosis and response to treatment, and understanding why some cancers are resistant to drugs, to develop new treatments.
PANCAIM is a European consortium to improve pancreatic cancer treatment with artificial intelligence, optimising and integrating genomics and medical imaging. Firstly, to help generate breakthrough knowledge to increase understanding of pancreas cancer biology. Secondly, to develop trusted, impactful AI applications for regular clinical use. The aim is to help clinical decision-makers to give the right treatment to the right patients at the right time. The aimed effect is to improve treatment outcomes of pancreas cancer patients avoiding the current costly trial-and-error use of expensive drugs with strong side-effects. AI is currently transforming the field of healthcare. Worldwide interest in AI is high and snowballing, fuelled by the availability of large digital datasets (“big data”). AI is the current most promising technology to integrate and optimize evidence-based decision tools. Main steps of the project are:
- Develop PANCAIM digital platform integrating genomics and clinical PDAC data
- Develop and use unimodal AI biomarkers for integrative research
- Develop and select most promising AI-assisted clinical products integrating omics and medical imaging data
- Implement and validate clinical products
- Sustain the platform for further research and clinical applications
Pancreatic cancer is also a research theme in the CRUK Scotland Centre. The work from this theme aims to understand the signalling networks between tumour and surrounding microenvironment, and how these might drive subtype development. The overall ambition is to facilitate a platform representative of human disease, encompassing multiple subtypes and stages of disease, for therapeutic testing.
Team SAMBAI is tackling the cancer inequalities challenge (Social, Ancestry, Molecular and Biological Analysis of Inequalities) through a Cancer Grand Challenge award. Inequities in cancer prevention, screening and treatment lead to disparities in cancer incidence and mortality and are major public-health challenges. Because the relative impact of the different drivers of cancer in these populations remain unclear, approaches aimed at reducing inequities remain inadequate. The team aims to combine social determinants of health with genomics, immune profiling, and exposomics to tackle cancer inequities at an unprecedented scale. Their goal is to identify the underlying causes leading to disparate outcomes – and use this knowledge to drive much-needed change. The pancreatic cancer theme in SAMBAI aims to use well-annotated patient samples, routinely collected health care data and patient-derived pre-clinical model systems to understand the complex dynamics of tumour-host interactions in pancreatic cancer and identify novel therapeutic approaches with candidate biomarkers that can be tested in clinical trials.
Use of mouse models are extremely important in understanding the diagnosis and progression of pancreatic cancer. Mouse models are used to investigate the importance of mutations found in human pancreatic tumours. Models also enable the profiling of different genetic subsets of pancreatic cancer to better understand the disease and identify specific targets for therapy. In addition, it is also possible to undertake preclinical trials of targeted therapies in clinically and genetically relevant pancreatic cancer mouse models.
