The role of Myeloid cells in tissue homeostasis, inflammation, and disease resolution

The research in our lab aims to dissect the active roles of tissue-resident macrophages and dendritic cells in the joints, skin, and lungs. We focus on (a) their roles in maintaining healthy tissue homeostasis, (b) understanding their involvement in the pathology of rheumatoid arthritis (RA) and psoriatic arthritis (PsA), and (c) reinstating tissue homeostasis during sustained disease remission. While investigating the basic biology of these cells, we seek mechanisms that can be therapeutically harnessed to restore tissue homeostasis or identify biomarkers that can help match patients to the most optimal current treatments targeting inflammatory pathways.

To maximise the impact of human tissue discovery, our lab is guided by the principle of translation—from bedside-to-bench and back in which human and mouse research strategies are interconnected for optimal understanding of novel tissue pathways and clinical innovation. We deconvolute the heterogeneity of myeloid cells in biopsies from patients at various stages of disease trajectories, spanning from the initiation of the disease to drug-free remission. This is achieved through the application of single-cell omics and spatial transcriptomics. The function of identified cell clusters is tested using unique biopsy-derived micro-cocultures and synovial tissue organoid systems. Subsequently, we conduct detailed mechanistic studies in purpose-generated transgenic mice. Collaborating with Prof Alivernini's clinical translational lab and Prof Jaume Bacardit's explainable artificial intelligence (AI) tools; we then assess the biomarker/therapeutic potential of our discoveries in experimental medicine trials (see Figure 1).

Our lab functions within FOREUM funded multicentre RA and PsA consortia:

Synovial Tissue Macrophages (STMs) (Figure 2): Our lab has discovered functionally distinct synovial tissue macrophage (STM) populations and highlighted the pivotal role of communication between tissue-resident myeloid cells and fibroblasts in shaping tissue niches that regulate activation of infiltrating inflammatory and autoimmune cells. In health and drug-free RA remission, specific clusters of synovial resident myeloid and stromal cells together maintain homeostasis. For instance, lining-layer MerTKposTREM2pos synovial macrophages (STM) with a regulatory phenotype and lubricin-producing (CLIC5posPRG4pos) fibroblast-like synoviocytes (FLS) form a barrier against inflammation and aid joint mobility while interstitial MerTKposLYVE1pos STMs are located around blood vessels and control the influx of inflammatory cells.

Experimental mouse studies and human studies with early undifferentiated arthritis (UPA) suggest that the disruption of the MerTKposTREM2pos STM layer or pathogenic changes in their phenotype to TREM2low, with decreased expression of the regulatory pathways (e.g., VSIG4 and retinoic acid pathways) and an increase in inflammatory SPP1, may facilitate the onset of arthritis. This phenotypic shift in TREM2pos STMs creates an inflammation-permissive environment, enabling the initiation of synovitis associated with the influx of neutrophils and monocyte precursors of tissue infiltrating, highly inflammatory macrophages (e.g., MerTKnegSPP1pos and MerTKnegS100A12pos). This leads to a change in the lining layer FLS phenotype into MMPs/RANKL-producing joint-destructive phenotype and a selective expansion of CD90pos sub-lining FLS clusters (e.g., CXCL12posMHCIIpos) that drive chronicity of tissue inflammation. The transition from active arthritis to sustained remission that does not flare is associated with the restoration of the regulatory phenotypes in MerTKposTREM2pos and MerTKposLYVE1pos STMs that mediate tissue repair responses in stromal cells.

We are currently studying the origin and niche signals that drive the development of various STM clusters in health, RA, PsA and drug-free remission as well as the role of STM specific pathways (e.g., MerTK and VSIG4) in maintaining and reinstating joint homeostasis in drug free remission with the prospect of pinpointing novel therapeutic targets and/or biomarkers of disease trajectories.

Synovial Tissue Dendritic cells (DC). DC in tissue and draining lymph nodes play a vital role in driving adaptive immune responses against pathogens while balancing immune tolerance versus autoimmunity. Thus, a better understanding the biology of tissue dendritic cells can help to re-instate immune tolerance in autoimmune diseases. We recently established that the synovium contains predominantly myeloid DCs comprising 15 transcriptionally distinct clusters. Their relative proportions, spatial distribution, and functions differ between health, active RA and RA in disease remission (in press). We developed specific methods and tools to investigate how these distinct synovial tissue DC clusters develop and regulate antigen-specific autologous lymphocytes. These include synovial tissue organoid system that enables tracking the developmental trajectories of specific synovial DC clusters from their blood precursors. In addition, autologous blood T-cells (from healthy and RA) CRISPR-Cas9 engineered to express TCR specific for multiple RA citrullinated-autoantigens enables us to assess the T-cell stimulatory versus tolerogenic functions of different autologous synovial tissue DC clusters.

Current RA treatments, while reinstating some of the mechanisms controlling inflammation in drug-free remission, don't reinstate immune tolerance thus we are currently investigating how to harness synovial tissue tolerogenic DC clusters to progress drug-free remission towards reminiscence of cure.
