General guidance on referrals to Occupational Health is contained within Section 5.4 of the Managing Attendance Policy.  In order to refer a member of staff to occupational health you should complete the form below using the accompanying guidance notes.

Line managers should ensure that members of staff receive both the form and the FAQs when being contacted to attend an Occupational Health review.

Completed forms should be sent to the relevant local HR Team.
