PERFORM Trial: Personalised exercise-rehabilitation for people with multiple long-term conditions

Do you have two or more long term health conditions? Would you like to take part in an exercise-based rehabilitation trial?
We are looking for people with two or more diagnosed long-term health conditions e.g. osteoarthritis and depression or you had a stroke and also have high blood pressure.
Taking part will involve attending your local study site for 3 extra research visits lasting approx. 2 hours.
- Baseline Assessment Visit
- PERFORM rehabilitation programme involving 2 hour group sessions twice a week for 8 weeks. These involve group exercise (warm up, strength, aerobic, cool down) followed by health and wellbeing presentations on different topics.
- 3 month follow up visit
- 12 month follow up visit
The study will inform future NHS and community provision of exercise rehab services.
