Dr Robert Sykes

  • Clinical Lecturer in Ischaemic Heart Disease (School of Cardiovascular & Metabolic Health)

Biography

Current Position

I am a Clinical Lecturer in Ischaemic Heart Disease in the School of Cardiovascular & Metabolic Health, appointed in 2024 through the Scottish Clinical Research Excellence Development Scheme (SCREDS), and a Specialty Registrar in Cardiology and General Internal Medicine in the West of Scotland. My clinical work is currently based at the Queen Elizabeth University Hospital in Glasgow.

Research

My research focuses on  novel diagnostics, myocardial injury with non-obstructive coronary arteries (MINOCA), Takotsubo syndrome and coronary microvascular dysfunction and the conditions it contributes to: angina and ischaemia with non-obstructive coronary arteries (ANOCA/INOCA). I am interested in stratified medicine, using coronary physiology and cardiovascular imaging to identify the mechanism behind a patient’s symptoms and match treatment to it, and in software that turns routine heart imaging into physiological measurements.

Current work includes analysis of the StratMed-MINOCA trial and a 7 Tesla MRI study of functional and structural brain abnormalities in Takotsubo syndrome, which I lead with support from NHS Greater Glasgow and Clyde Endowment Funds. I also contribute to the group’s wider programme, including the CorMicA-PCI trial and the international MICRO-SNAPE study of ANOCA endotypes (European Heart Journal, 2026), and to the NIHR-BHF MINOCA and myocardial injury workstream.

Background

I studied medicine at the University of Edinburgh (MBChB, 2010 to 2016), with an intercalated BMedSci (Hons) in Epidemiology and Biostatistics (2012 to 2013). I moved to Glasgow in 2016 for the Academic Foundation Programme at Glasgow Royal Infirmary and the Queen Elizabeth University Hospital, completing a PGCert in Health Professions Education at the University of Glasgow. I then entered Core Medical Training at Glasgow Royal Infirmary within the Glasgow Academic Training Environment, having gained MRCP(UK) through the Royal College of Physicians and Surgeons of Glasgow in 2018.

In 2020 I treated patients admitted with COVID-19 in the acute assessment unit at Glasgow Royal Infirmary and became involved in COVID-19 clinical research. That August I joined Professor Colin Berry’s group as a clinical research fellow, investigating the multisystem effects of COVID-19 and helping to lead recruitment of hospitalised patients to CISCO-19, RECOVERY and related studies. This work formed my MSc(Res) thesis (2022) and was published in Nature Medicine, where I was joint lead author.

From 2023 to 2026 I undertook my PhD as the clinical research fellow for StratMed-MINOCA, a registry-based diagnostic study and nested randomised, open-label trial in people with suspected MINOCA. Coronary microvascular function was measured invasively at angiography, and participants with a raised index of microvascular resistance were randomised to eplerenone or standard care. The study enrolled 379 participants and was supported by research funding from Abbott; its design was published in the American Heart Journal in 2026 and study follow-up concludes in November 2026. (PhD thesis submitted 2026). During both research fellowships I worked on the cardiology fellow rota at the Golden Jubilee National Hospital.

This training gave me experience across acute cardiology, cardiovascular imaging and mechanistic vascular science, including wire myography, cardiovascular magnetic resonance, coronary CT angiography, echocardiography, and invasive coronary angiography with coronary physiology.

Innovation

I write software in Python and JavaScript and use machine learning to build diagnostic tools. The most advanced is the VirtualIMR algorithm and Coroview platform, which estimates coronary microvascular resistance from a routine coronary angiogram without a pressure wire and is the subject of UK and international patent applications. Its development has been supported by the Medical Research Council Impact Acceleration Account and two rounds of the University of Glasgow’s MedTech Innovation Fund, and in 2025 I took part in Innovate UK’s ICURe Discover programme.

Teaching and collaboration

I teach undergraduate medical students, supervise BSc, MSc and student selected component projects, and examine in undergraduate OSCEs. I welcome enquiries about student projects and collaborations in ischaemic heart disease including coronary microvascular disease, MINOCA and Takotsubo syndrome, myocardial injury, heart failure and from clinical or industry partners interested in angiography-derived coronary physiology.

Research interests

My research focuses on advanced cardiovascular diagnostic techniques, including coronary physiology, echocardiography, intravascular imaging, and cardiovascular MRI, to address myocardial injury and ischaemia without obstructive coronary disease.

I am dedicated to integrating emerging technologies such as AI and machine learning into clinical practice, aiming to optimise patient outcomes and advance population-level healthcare solutions.
