Improving Secondary Prevention after Stroke in People with Multiple Long-Term Conditions from Areas of Socioeconomic Deprivation

Summary  

Cardiovascular and metabolic multimorbidity complicates the long-term optimisation of guideline-directed medical therapy (GDMT) and secondary prevention following a stroke, particularly within socioeconomically deprived populations. This project investigates the clinical and operational drivers of sub-optimal prescribing and target non-attainment.

Utilising multi-linked electronic health records from the longitudinal SPORT cohort, the candidate will apply natural language processing to extract documented adverse drug reactions and intolerance narratives from unstructured clinical text. Concurrently, longitudinal trajectory modelling will analyse serial biochemical markers alongside prescribing records.

Chronologically integrating these data streams will establish multimodal patient phenotypes to map treatment-attainment pathways. Working alongside health economists, these clinical profiles will populate a decision-analytic framework to evaluate the cost-effectiveness of stratified prescribing strategies for advanced therapies.

The candidate will be supported to develop robust quantitative skills in pharmaco-epidemiology, health technology assessment, data governance, and public-patient involvement. This project suits an applicant interested in clinical pharmacology, health data science, and secondary prevention.
