Biography

I am a Consultant Trauma and Orthopaedic Surgeon and Honorary Clinical Associate Professor, working at the point where orthopaedic surgery meets cell engineering, bacteriology and materials science.

My clinical practice in NHS Ayrshire & Arran and at the Queen Elizabeth University Hospital covers complex primary and revision hip and knee arthroplasty, prosthetic joint infection, metastatic bone disease and salvage orthopaedics. I established and lead the West of Scotland service for metastatic bone disease and the Ayrshire & Arran bone and joint infection multidisciplinary team.

That practice is also the engine of my research. I completed my PhD in the Centre for the Cellular Microenvironment, developing a human bone marrow co-culture model to test how osteoblasts and osteoclasts respond to engineered implant surfaces, and showing that strontium-eluting controlled-disorder nanotopographies drive bone formation while suppressing osteoclast activity.

I remain embedded in CeMi, and I work with the School of Infection & Immunity on the biology of biofilm-driven implant failure and with colleagues in Molecular Biosciences on the synovial glycome in inflammatory arthritis. I supply the patients, tissue, synovium, bone marrow and retrieved implants that these programmes depend on, and I take the findings back to the operating theatre.

I direct the Glasgow Orthopaedic Research Initiative (GLORI), which brings surgeons, scientists and engineers together across the University, the University of Strathclyde and NHS orthopaedic units. I am a local principal investigator on national randomised trials in metastatic bone disease and inflammatory arthritis, and I build research partnerships with the orthopaedic implant industry.

I completed fellowship training in Vancouver as the BOA ZimmerBiomet and Gamada Ayana travelling fellow, and at the Royal Orthopaedic Hospital, Birmingham, in arthroplasty, bone infection and orthopaedic oncology.

Research interests

Implant-associated infection and biofilm. Why prostheses fail through infection, and what the organisms actually do on the implant surface. I work with the School of Infection & Immunity applying cross-scale spatial biology, metagenomics and multi-omics to retrieved human prostheses and periprosthetic tissue.

Engineered surfaces and bone regeneration. Nanotopography, strontium elution and mechanotransduction as tools to make implants osteoinductive and anti-osteoclastic, using human bone marrow co-culture models.

The bone marrow niche in cancer and dormancy. Clinical direction and human tissue for CeMi programmes modelling the niche, including bioengineered platforms for testing cell therapies.

The synovial glycome. Synovium and matched clinical data to investigate glycosylation in the interplay between systemic and local inflammation in inflammatory arthritis.

Metastatic bone disease. Surgical reconstruction, implant survival and the evidence base for how and when to intervene, including national trial recruitment and registry-scale outcome work.

Supervision

I welcome enquiries from clinical and non-clinical applicants interested in:

  • Prosthetic joint infection: biofilm architecture on retrieved implants, spatial biology, and why culture-negative infection happens
  • Antimicrobial resistance in polymicrobial orthopaedic infection
  • Engineered implant surfaces: nanotopography, ion elution and the osteoblast–osteoclast balance
  • Human tissue models of the bone marrow niche
  • Metastatic bone disease: surgical reconstruction, implant survival and outcome prediction
  • Clinical trial and registry work in revision arthroplasty

I can offer access to a substantial clinical cohort, human tissue, retrieved implants and theatre, alongside laboratory co-supervision within CeMi and the School of Infection & Immunity. Clinical trainees considering an MD or PhD are particularly welcome to get in touch.
