SummaryFull report

The SGHD-funded Govan Integrated Care Project (SHIP) aims to improve integrated care for patients served by the four general practices at the health centres via additional GP capacity, attached social workers, attached link workers (2 practices), support for monthly multidisciplinary team meetings in each practice to review vulnerable families and frail elderly patients.

Dr John Montgomery, GP lead for the SHIP project, described the project in a presentation to the 3rd National Deep End Conference in November 2015.
