Health Declaration Forms

All student entrants to Medicine, Dentistry, Nursing and Veterinary Medicine must complete an Occupational Health Declaration form. 

More information can be found in the relevant College or School website for your programme such as the College of Medical, Veterinary and Life Sciences home page.

We are happy to receive forms electronically by email ohu@admin.gla.ac.uk or, by post (you must ensure you have sufficient postage if you are sending this to us): Occupational Health Unit, 63 Oakfield Avenue, Glasgow, G12 8LP.  You should keep a copy of the completed form for your own records.

Medical, Dental and Nursing students must complete the detailed consent form attached to the health questionnaire and return this along with the completed health declaration form, as above.   

Information for students undertaking ELECTIVE placements

The Occupational Health Unit is happy to provide you with a copy of your vaccination record and confirmation of your Blood Borne Virus (BBV) status, if you have not already received a copy of this from us.

Some students may require up-to-date repeats of BBV tests for placements in certain countries. As the University of Glasgow has already carried out these tests for you on entry to 1st year, you are resopnsible for the cost of repeating them. For up to date costs please contact the Occupational Health Unit  

 

 

 

 

 

 

 

 

 

 

 

 

 

 

Please remember that all of these tests take time to arrange and return to us so you must book them in plenty of time (at least 4 weeks) before you depart.  Your elective placement may ask that blood tests are carried out within a specific period of time (ie within two months of your placement start date) and we would ask that you consider this before making an appointment with us.   

If you need a chest X-ray it will cost around £70 at Gartnavel General Hospital (X-ray Unit). You need to first obtain an X-ray request form from Mrs Nicola Cumming, at the Medical School, who will advise you to take this to the Gartnavel General Hospital cash office (the staff in the X-ray Unit will point you in the right direction).

If you believe you have had exposure to Blood Borne Viruses whilst on an elective placement abroad it is your professional responsibility to report this in confidence to the Occupational Health Unit immediately on your return.

OH Immunisation Records - Undergraduate Medical, Dental and Nursing Students 

At the end of your first year you should have completed your full immunisation schedule.

When you have completed your immunisations we will send an electronic copy of your immunisation record to your University email address.

The information contained within these records is important as you will be asked for this record once you graduate and begin your career in Medicine, Dentistry or Nursing.

It is your professional responsibility to keep these records in a safe place for future use.  If we are required to produce a replacement set of these records, there will be a £50.00 charge to do so and this must be paid before we can issue a second copy of your records.
