Occupational Health Reviews

General guidance on referrals to Occupational Health is contained within Appendix 5 of the Sickness Absence Policy.  In order to refer a member of staff to occupational health you should complete the form below using the accompanying guidance notes.

Line managers should ensure that members of staff receive both of the above forms when being contacted to attend an Occupational Health review.
