Professor McMillan has been Professor of Clinical Neuropsychology at the University of Glasgow since 1999. He is also adviser to NHS Greater Glasgow and Clyde with regard to service development for brain injury and provides a service assessing extra-contractual referrals for them. He is Director for postgraduate education in Mental Health and Wellbeing and is Research Director for the MSc and Doctoral programmes. He is President of the International Brain Injury Association.  

His research focuses on the epidemiology and outcome of brain injury, including factors that might explain the heterogeneity of outcome and late mortality and in addition trials of psychological interventions and neurorehabilitation. He has been involved in the development of SIGN Guidelines for Early Management of Head Injury and Rehabilitation of Brain Injury.

He is an associate editor of Neuropsychological Rehabilitation and until recently on the editorial board of Brain Injury.
