Candidates should have graduated with their first degree no more than 15 years ago and have made a significant contribution to their chosen field.

  • Name, Address, Degree & Year Information for the Candidate
  • Name, Address, Degree & Year Information (if relevant) for yourself
  • Reason for nomination
  • Supporting evidence - e.g. newspaper cuttings, web biographies, CVs
